A. You will need a Caregiver Form (available in the office) if you are “temporarily” being cared for by someone other than your parent or legal guardian. The Caregiver Form is ONLY TO BE USED in the event a parent is incarcerated, terminally ill or out of the country (temporarily). Other cases would require a court appointed guardianship thru the court system.

Q. My parent and myself reside with someone else. We don't have a gas or electric bill with our name on it, what do I do?

A. You must have a Residency Form (available in the office) on file. The person with whom you and your parent/guardian are living with MUST fill out this form. You must also provide a gas/electric bill showing the name of the person with whom you are renting a room from.

 

A. You must meet with an Administrator to be sure you are on track to return to Katella. If you are coming from Community Day School etc. you should have met with your Assistant Principal or Counselor there and given a copy of your transcripts indicating you are now on track to graduate. Please come prepared!!

Q. I can pick up my packet but I will be gone during the dates to return the paperwork. What can I do?

A. Return your paperwork when you get back from vacation. Our master schedule is already set up and your classes are already in place. There won’t be a problem with your schedule because you are on vacation and unable to attend on the appropriate day.
