Athletic Eligibility Packet

Students that want to participate in athletics must complete and return all of the following forms. Students must be cleared through the athletic department before participating in practice or games, and must carry a 2.0 gpa.

2. Emergency Health and Medical Consent Form (2 copies)
3. Physical Preparticipation Evaluation (2 pages)
4. Signature Page

**Students must have the physical form signed by MD or DO
**Students must have valid insurance
